AGE 8-39 – Athlete

Download the Athlete Physical and take to the Doctor
Make sure the Doctor PRINTS their name and stamps the form

AGE 40-44 – Master Athlete

Download the Masters Athlete Physical and take to the Doctor
Make sure the Doctor PRINTS their name and stamps the form

Age 45+ – Master Athlete

Download the Masters Athlete Physical and take to the Doctor
Make sure the Doctor PRINTS their name and stamps the form
Boxers over the Age of 45yo are also required to have a graded exercise EKG every 5 years
